New Vegas added a bit of the RPG element in the "hardcore" mode: having to eat, sleep, and stay hydrated. Ammo management came into play as well. I liked all of those features and I hope they bring them back.

Edit: It was nice to see a game ADD features of difficulty rather than continually dumbing down the sequels (a la The Elder Scrolls series...).

The power armor works like Exosceletons, hence the PA-Training, motion-assist servo-motors and the built-in Microfusion cells.

How about bringing back the limitation from old school games, where you are able to save your progress only at certain locations, like safehouses or paid hotel rooms? This feature alone would bump up the difficulty quite a lot. Because now whenever there's a feeling things can go wrong just save/load and eventually you will get over it.

It would be nice if they implemented a hard core mode in the next fall out game like Diablo 3, that if you die, that character permanently dies, maybe initiate it after the introductory chapter so you don't have to go through the boring linear part again

They have so many things right that to change up the difficulty would require some minor AND major design changes. I have a feeling they will up the ante and improve a decent amount from last time around
