Simple friend. It’s a double entendre. Where one meaning is obvious and the other is more subtle. The obvious being snake’s missing arm and the resulting medical phenomenon. The subtle being his past coming to haunt him. These “phantoms” are most likely delusions due to the giant shrapnel in his forehead. He is a man changed and after such traumatic events will see a rehashing of his past and a re-evaluation of his moral compass.

Just pointing out that Snake loses his arm in The Phantom Pain. You can clearly see his robotic arm in all trailer and gameplay footage revealed thus far 😛

There are indeed ghosts in the series (The Sorrow, MGS3), but jmmy wasn’t referring to the eye, but the lost arm.

*Coughs* He’s missing his eye. Plus the game has supernatural properties and Ghosts are literal easter eggs in every game.
