Tenchu: Stealth Assassins
Tenchu: Stealth Assassins is actually a pretty great game, it does everything right, from martial arts to the story, in fact it was one of the first game to incorporate innovative stealth mechanics into the game. Now aside from all the great gameplay, the moment you die you are really in for it, because the game really rubs it in. When you die, there is a message that flashes on your screen “the ninja Ayame returns the shadows from where she came, to be forgotten forever as a failure”. Video starts at 3:13.
