Mein Leiben! difficulty is only unlocked upon beating the game on “I am death incarnate!” Again, the difficulty between the two is the same but there’s no saving. You can’t manually save and auto-saving is also disabled. So you could die or even quit the game mid-way and you’d have no save data to go back to.
