True Ending – Painkiller
For a game named Painkiller, the amount of agony needed to get the final ending is unbearable. First, you have to beat the game on Nightmare. Now, beat each card condition on each level. Having fun yet? After unlocking all the cards, you get Trauma difficulty. Beat that and you’re on your way to the final ending.
