As confusing as the title is, the main confusion lies in the protagonist’s eyes. I mean seriously, if you take a look at the two guys in the box art, it seems at least to me like that patient and the inspector share some sort of weird relationship with each other. And from the looks of it, it seems as if that relationship is them sharing the same cell in an asylum.
