You’re actually playing as a medic who was transformed via plastic surgery to look like Big Boss, becoming Venom Snake through hypnotherapy. The real Big Boss reveals all this to you at the end via tape recording because we really needed a reason for how he managed to survive Metal Gear 1 and reappear in Metal Gear 2.
