8. What Floor?
Most of the fails here have been due to a lack of gravity but, in this example, there's far too much. The two most recent Fallout games are renowned for sporting a myriad of glitches, but there are times when you just can't beat a classic "fall through the floor" error. This guy seemed to be doing well with the flamethrower, but it looks like the extra weight of the weapon was just too much for gravity to handle.
